Emergency Response Checklist

As a reminder, The Joint Commission Tri-Annual survey is drawing near and you may be asked to share some of the following information on how you should respond to emergencies and disasters.

Here are five reminders when activating or responding to emergency or disaster:

  1. When deemed necessary, any employee can declare a Code Gray (no weapon).
  2. In a Code Silver (weapon), find the nearest phone and dial Security at 543-6603 or switchboard at x-6000. Identify yourself and your department, and advise you “have a code silver situation”.
  3. To make an emergency overhead announcement, locate the nearest telephone and dial ext. 7226 11#. Emergency announcements should be repeated twice.
  4. If a fire is in your department, remember these acronyms:
    R.A.C.E (Rescue, Activate, Contain and Extinguish/Evacuate)
    P.A.S.S. (Pull, Aim, Squeeze, Sweep)
    Only Security, Safety Management or Bethel Fire Department can designate the affected area “safe” to renter.
  5. A Red Emergency Response Guide is located in every department. Ensure you know its EXACT location.

  • Code Red
    (Fire Alarm Activation)
  • Code Blue
    (Medical Emergency)
  • Code Yellow
    (Bomb Threat)
  • Code Orange
    (Hazardous Material Spill)
  • Code Pink
    (Infant Abduction: 0-12 months)
  • Code Purple
    (Child Abduction: 12 months)
  • Code Gray
    (Civil Disturbance: No Weapon)
  • Code Silver
    (Civil Disturbance: Weapon)
  • Code Evacuation
    (Evacuation of facility/department)
  • Code Triage External
    (Patient Surge)
  • Code Triage Internal
    (Internal Diaster)
