The 2021 Culture of Safety Survey was launched April 16, 2021. The survey is designed to collect perspectives regarding delivery of services in current working conditions from current staff. This survey is important for the corporation because it gives employees the opportunity to provide feedback on our current culture of safety in the workplace.

The survey will be extended to May 7, 2021 in order for more responses to be submitted. Our goal is to collect a minimum of 300 employee responses. As of April 28 we have received a total of 210 responses from the following departments

The survey is completely anonymous. A separate survey is available for those interested in entering a raffle for Amazon Gift Cards and YKHC swag provided by the Performance Improvement Department.
