Safety Message: Continuation of YKHC Environment of Care (EOC)Tour questions

This week’s safety message overview of EOC questions is for nursing questions and observation. Below are the FAQs

  1. Crash cart locked with tag intact, ready for use and current log.
  2. Medications locked or under constant surveillance.
  3. All medications labeled and not expired.
  4. Opened multi-dose vials initialed & marked with date opened.
  5. No open multi-dose expired (28 days).
  6. All supplies are not past the expiration date.
  7. All drugs needing refrigeration are stored properly.
  8. No food or open food containers at nurses station.
  9. Refrigerator thermometer (36-40⁰F) and documented on daily log.
  10. What are five rights of medication administration?
    patient, route, medication, time, dose
  11. How to identify patient prior to medication? Always need two
    ask name, social, birthday or medical record #; check wristband
  12. Can staff verbalize or identify unapproved abbreviations?
  13. Where is list of unapproved abbreviations located?
  14. Nurse: verbalize process of receiving critical test result?
    repeat results back with ID, write down, alert provider or house supervisor
  15. Nurse: what to do if “adverse drug event” occurs?
    Stop medication, assess patient, notify physician, list as allergy in EHR
