Receiving HealthStream alerts? An important performance evaluation reminder

All performance evaluations need to be finalized no later than Friday, March 26. If you are receiving reminder emails from HealthStream, your performance evaluation has not been finalized.

There are several steps that need to be completed in order for the evaluation to be considered complete. Please review the step-by-step process below and the assessment guides:

A snapshot of YKHC’s 2020 Performance Evaluations as of today, March 18:

  • 12% are in the Approval State
    Performance Evaluations are under review by HR or the Manager’s Manager
  • 26% are in the Evaluate State
    Performance Evaluations have not been submitted by Employee and/or Manager
  • 40% are in the Review State
    Performance Evaluations have been reviewed & approved. In the Review State the Manager & Employee need to meet one-on-one to sign and finalize
  • 22% are Closed & Finalized
    All steps have been completed

Step-by-Step Process:


All employee self-evaluations and manager evaluations are due and need to be submitted ASAP. As outlined in YKHC’s employee performance evaluation Policy HR_005, each employee is required to have annual performance evaluation.

Please Note: HealthStream can be accessed from anywhere via www.healthstream.com/hlc/ykhc. If you have not already done so, please log-in to HealthStream today to complete YKHC’s 2020 performance evaluation.

Managers: Please reference your email for additional information from Human Resources.

The purpose of the performance evaluation is to review expectations, employee goals, and to assist employees in reaching their full potential. Our intention is to assess our employees’ achievements in relation to their job description and against organizational, team, and individual performance objectives. As we work together to achieve excellent health across the YK-Delta, YKHC is focused on creating a work culture where employees feel valued, safe and respected. Leaders care about what employees think, and we seek our employee involvement in all aspects at YKHC. Our annual performance review is an opportunity for employees and managers to reflect on the many contributions, solutions and accomplishments throughout 2020. 

Key Dates:

  • All Employee Self-Evaluations are now due
  • Manager Evaluations are now due
  • All Evaluations must be signed and finalized no later than Friday, March 26
