Refresher: How to add Family to Benefits

Adding family to you benefits:

If you are enrolled in YKHC benefits, such as health insurance, dental/vision insurance, Flexible Spending Accounts and/or Optional Life insurance and you want to enroll a family member in your plan, keep in mind there are only two instances that you may do so: a Qualifying Life Event or Open Enrollment. 

If you are experiencing a QLE, you can add immediate family members (spouse or children up to age 26), but there is a time limit (most are 60 days) to submit the documents to Human Resources. A QLE is an event such as: getting married, getting divorced, having a baby, adopting a baby or your spouse loses insurance coverage. In addition, every year, Open Enrollment is provided from November to December, an opportunity for employees to add immediate family members to a plan, drop a plan, change plans or enroll for the first time. Changes made during Open Enrollment are effective January 1, and employees stay on the plan until he/she drops it, quits or becomes non-benefitted.

If you are participating in YKHC’s retirement plans with OneAmerica, we encourage you to log in to the https://oaretirement.com website and set up beneficiaries for your 963241 (401a employer match account) and 963242 (403b deferral account) accounts, if you haven’t done so yet. If you are married, the IRS requires married enrollees to list his/her spouse as a beneficiary, unless the spouse waived this in writing. 

If you are Regular Full Time with Benefits or Regular Part Time with Benefits, you are covered automatically for the Employer Sponsored Life and Accidental Death and Dismemberment Policy with Unum, the carrier. Your employer pays your portion of your premium. You were asked during New Employee Orientation to fill out a Unum Beneficiary Designation form. You may update this at any time, by downloading a copy from the document library or MCN, fill it out completely and send it to HR benefits staff. If you are enrolled in Optional Life insurance, or Critical Illness Insurance, you may update your beneficiaries by completing the beneficiary designation form, and submitting it to HR benefits staff.
