Culture of Safety survey in review

The annual Culture of Safety Survey was open from April 16 to May 7, 2021. The Culture of Safety Survey provides feedback on YKHC’s communication, teamwork, patient safety, leadership, and staff engagement. The anonymous results of the survey were shared with the Senior Leadership Team (SLT) on May 17 and will be shared during this month’s Quality Assessments/Performance Improvement (QAPI) meeting. Department specific results will then be shared with employees by their leaders.

We received a total of 398 responses, with 251 raffle entries for free swag! All raffle entries were placed in an on-line randomization program.

Congratulations to the following employees who were selected by that program as winners in the 2021 Culture of Safety Survey $25 Amazon Gift Card Raffle:

  1. Lacy Stout, Surgery
  2. Allison Ayapan, Dental
  3. Keira Gefroh, Physical Therapy
  4. Virginia Howard, Non-patient care area,
  5. Ronald Fern, Radiology
  6. Lori Chikoyak, Non-patient care area,
  7. Angela Jackson, outpatient
  8. Christie Andy, outpatient

For those employees who entered the raffle portion of the survey and didn’t win a gift card you, will not walk away empty handed. Each individual will receive a swag bag.

Please contact any member of the Performance Improvement Department (Debra Samson, Jessica Pew, John Young, or Joseph Okitkun) by email or phone with your name, address, and phone number so that we can get your swag to you!

We would also like to thank all employees that participated in this survey. This participation allows your voice to be heard and allows YKHC to:

  • Raise staff awareness about patient safety.
  • Assess the current status of patient safety culture at YKHC.
  • Identify strengths and areas for patient safety culture improvement.
  • Examine trends in patient safety culture change over time.
  • Evaluate the cultural impact of patient safety initiatives and interventions.
