A reminder about process for adding and revising forms

Did you know that every YKHC “form” should have a PR-assigned document number and revised date in the lower right corner?  And be loaded into MCN/Policy Manager?

For the purposes of this article, a “form” is any document used on a regular basis as part of standard work for one or more of the following purposes:

  • Collect customer or employee information
  • Obtain legal signatures
  • Track information or record data
  • Order supplies

YKHC has hundreds of forms in use throughout our organization. The most updated version of any form should be available on our MCN Policy Manager, which can be found on the intranet.* Having one central location for all approved forms ensure a person can always find the most updated version of a form, which improves patient care and maintains compliance with Centers for Medicare Service and Joint Commission requirements.

As systems evolve, forms need to be updated.*  It is the responsibility of the department manager to ensure the most updated version of any forms they manage are available on our MCN Policy Manager.

Before a form is uploaded, it must be created or updated by the Public Relations department, who will assign the form and updated document number and revised date in the lower right corner.Requests for new or updated forms should be submitted through the PR Service Request System, which can be accessed from the home page of the intranet.

Once the form is approved, follow the instructions for “How to Upload a New Form” into Policy Manager.

Then your new form will be official!

* Please note: There are other types of documents that should be on MCN – not only forms. For example, standard work, policies, procedures,, etc. also need to go in MCN. If you have questions about if a document you use regularly should be in MCN, email performanceImprovement@ykhc.org.

*If you are using any form that has the old YKHC logo (triangle, blue and yellow, with goose) rather than the new logo, that form needs to be updated with proper branding. Please submit the form for rebranding in the PR Service Request system.
