Fill out annual Culture of Safety Survey and be entered into a raffle drawing!

This survey is not only a Joint Commission requirement, but is useful for measuring organizational conditions that can lead to adverse events and patient harm. The safety culture survey is used to:

  • Raise staff awareness about patient safety.
  • Assess the current status of patient safety culture.
  • Identify strengths and areas for patient safety culture improvement.
  • Examine trends in patient safety culture change over time.
  • Evaluate the cultural impact of patient safety initiatives and interventions. 
  • Allow all YKHC employees the opportunity to provide feedback on current workflows in units/departments.

To be considered valid, the survey will remain open until it has at least 300 employee responses.

The survey will allow YKHC employees the opportunity to provide feedback on current workflows in each of their units or departments. YKHC is dedicated to providing high reliability in health care and a safe work environment, and your feedback is greatly appreciated. Door prizes will be available throughout the survey period such as gift cards.
