Safety Message: New way to request badge access

Department supervisors and managers can now request badge access for employees submitting a request via the “Proximity Card Access” button on the home page of the intranet. (See image at the bottom of this article.) Please discontinue use of any Proximity Card Access forms and please do not email the Security-BadgeAcess@ykhc.org for access.

Access levels should only be requested and approved by supervisors and managers.

Please click on the “Proximity Card Access” link and make sure to attach a clear, copy of the employee’s ID badge.

An important note: any OB access requests will require prior OB nurse manager approval, as OB is a secured unit.

Employee Badges must be worn at all times by staff, residents, interns, students, and contractors with the picture and name visible so they can be readily seen by security personnel, patients, visitors, etc. Badges may not be loaned, transferred, or used by anybody other than the person who is pictured and named on the badge. Your employee ID badge is your electronic key to enter the facilities and other secured areas as needed.

Please Immediately contact security (x 6603) to deactivate any lost or stolen badges. If an employee ID badge is found, please turn it into security immediately. If you lose your badge, human resources can create and replace your employee badge for a $10 fee.
