As Employee COVID-19 testing has began at YKHC, a log has been created for managers to use when their department tests. This log is to list employees, whether or not they have tested, and whether or not they are exempt from testing (if the employee already had a POSITIVE COVID-19 test they are exempt from testing for 90 days).

When you access this file it is set to auto download for printing as excel the spreadsheet will not show correctly in the system. It will however print properly to then fill out.
